- Medication management

Assisted Living II
- Limited assistance from team member for medication management
- Limited management of chronic conditions (e.g., oxygen, treatments, injections, TED hose)
- Stand-by assistance for ambulation
- Transfer assistance if needed
- Meal set-up, cueing
- Stand-by assistance for bathing safety
- Minimal assistance with personal hygiene and dressing
- Minimal assistance with toileting
- Cueing, meal and activity reminders

Assisted Living III
- Extensive assistance from team member for medication management
- Extensive management of chronic conditions (e.g., oxygen, treatments, injections, TED hose)
- Assistance with ambulation
- Transfer assistance if needed
- Meal set-up and cueing, minimal assistance with eating
- Hands-on assistance with bathing
- Moderate assistance with personal hygiene and dressing
- Moderate assistance with toileting
- Daily reminders for meals and activities
Assisted Living IV
- Fully dependent on team member for medication management
- Fully dependent on team member for management of chronic conditions (e.g., oxygen, treatments, injections, TED hose)
- Unable to ambulate without wheelchair
- Requires two-person transfer, mechanical lift, or hospital bed
- Must be fed by team member
- Dependent on team member for bathing, personal hygiene, and dressing
- If continent, requires team member to toilet every time; if incontinent, requires team member to manage incontinence
- Requires team member intervention to manage resistance to care
