Now Hiring: Lakewood at Home Care Coordinator

The Lakewood at Home team is expanding! Due to the success and interest in Lakewood at Home, our membership is growing, and the team is seeking an enthusiastic, experienced senior care professional to be an additional Care Coordinator for our members.

Lakewood at Home’s Care Coordinators are responsible for ensuring that the delivery of care, programs, and services to Lakewood at Home members are provided with the same outstanding quality the residents of our Lakewood campus receive. The Lakewood at Home program will include long-term care programs, transportation, care coordination, access to local resources, and other ancillary services. They maintain a working knowledge of resources for seniors available in our market, manage the delivery of care to our members, and manage relationships with credentialed vendors. They work closely with our Sales Counselors to address the needs of prospective, new, and existing members.

The selected candidate will support the existing Care Coordinator in coordinating services for this membership program. The ideal candidate will be a social worker or licensed nurse with clinical assessment skills and will preferably have experience working with seniors. Bachelor’s degree in a related field is preferred.
